Therapeutic Approach of Peritoneal Carcinomatosis in patients with Colorectal Carcinoma

Abstract

Peritoneal carcinomatosis is an advanced stage of colorectal cancer, with poor prognosis, that has been regarded as an untreatable condition, suitable only for palliative treatment. Over the last decades new treatment options have emerged; cytoreductive surgery in combination with intraperitoneal chemotherapy has increasingly gain acceptance as a treatment for patients with colorectal peritoneal carcinomatosis. Recent clinical studies have suggested that this new approach could result in long-term survival in selected patients. The emerging trend is to regard peritoneal carcinomatosis as a regional metastatic disease, suitable to locoregional treatment. However a successful outcome depends on an appropriate selection of patients: patients with limited peritoneal disease, where complete cytoreduction is achieved have a greater benefit with this new therapeutic strategy. This review sumarizes indications, technical aspects, limitations, outcomes and challenges of this new approach in the treatment of colorectal peritoneal carcinomatosis.
